Please fill out the Employer 1 form. Once you've completed it, you'll be directed to the Employer 2 form. If you have a second employer, please fill out that form as well. If you don't, just skip it. If you have a third employer, after submitting the Employer 2 form, you'll be directed to the Employer 3 form. Please complete that as well. For any additional employers, just repeat the process.Please read all the instructions before proceeding to answer each question. There is an instruction (if applicable) in the boxes on how to answer the questions. After completing EACH form, please click "SUBMIT".  
Silakan isi formulir Employer (Majikan) 1. Setelah anda menyelesaikannya, anda akan diarahkan ke formulir Employer 2. Jika anda memiliki majikan kedua, isi formulir itu juga. Jika tidak, lewati saja. Jika anda memiliki Employer ketiga, setelah mengirimkan formulir Employer 2, anda akan diarahkan ke formulir Employer 3. Silakan lengkapi juga. Untuk majikan lain, ulangi saja prosesnya. Baca semua petunjuk sebelum menjawab setiap pertanyaan. Ada petunjuk (jika berlaku) di kotak tentang cara menjawab pertanyaan. Setelah melengkapi SETIAP formulir,  klik "SUBMIT".
